Easing transitions
with hope

Dialyze Direct optimizes your journey from hospital to on-site dialysis for patients in skilled nursing facilities (SNFs). We partner with rehabilitation centers, residential care, and long-term care facilities. Our trained caregivers deliver on-site dialysis based on individual healthcare needs and schedules.

For residents needing regular treatment, dialysis in nursing homes can transform the care experience. Our innovative approach provides many benefits for both residents and their SNFs.

Health and Wellbeing

Renal services from Dialyze Direct are designed to improve residents’ quality of life. Residents who receive dialysis within the comfort of the nursing home are able to participate more fully in community activities, spend more time with loved ones, and avoid the stress of visiting a dialysis center. This can result in improved well-being and life satisfaction.

Nursing homes with dialysis on-site are better able to help with all aspects of a resident’s health. When residents can spend more time in their care facility, they gain greater access to therapy and other healthcare models. This can result in more positive health outcomes overall.

Treatment that Fits your Schedule

At Dialyze Direct, we understand the complexity of the nursing home environment. Our team is experienced in coordinating nursing home dialysis alongside residents’ other scheduled activities. At the same time, on-site SNF dialysis eliminates transportation time and improves patients’ access to nursing home services and amenities.

Coordinated Renal Care

We know the importance of quality, coordinated care for kidney health. Our caregivers work with each resident’s nephrologist to design an individualized treatment plan. In addition, our onsite nurses work directly with your rehabilitation team to coordinate care.

Specialized Dialysis Vascular Access Care

Many dialysis patients enter our program with a central venous catheter as their dialysis access. These catheters can become infected.  Therefore, Dialyze Direct’s clinical terms receive highly specialized catheter care training, resulting in dramatically lower catheter infection rates. The relative risk of catheter infections for Dialyze Direct patients is 70% lower than the industry catheter infection rate.  This is one important reason why Dialyze Direct patients have fewer hospitalizations and can more effectively participate in rehabilitation and recovery programs.

Frequency of Dialysis

At many of the facilities we partner with, patients are able to receive more frequent dialysis (MFD) (pursuant to a physician’s order). This treatment plan entails five treatments per week, each lasting less than three hours. This schedule is designed to streamline care. Studies suggest that MFD offers several clinical benefits:

– Shorter dialysis time: Each MFD treatment takes about 2 hours and 50 minutes.
– Fast post-treatment recovery: An impressive 94% of patients recuperate within two hours.
– More effective treatment: MFD supports better removal of fluids and toxins, and more stable blood chemistry.
– Improved blood pressure levels: Blood pressure is managed more effectively with MFD. Patients are often able to reduce their number of daily blood pressure medications as a result.
– Earlier discharge: Rapid recovery from dialysis permits more participation in rehabilitation therapy, allowing patients to return home sooner.

While an MFD treatment schedule would be challenging to coordinate off-site, Dialyze Direct’s renal services make it easy to deliver within the nursing home. Simply choosing Dialyze Direct can make a substantial impact on residents’ lives.

Support for Ventilator and Tracheostomy
Patients

Select facilities are equipped to provide comprehensive care for residents who need additional respiratory support, including ventilators or tracheostomies. The ability to offer dialysis and respiratory support in one location ensures that residents receive uninterrupted, coordinated care.

Enhanced Quality of Life

The primary aim of Dialyze Direct is to enhance residents’ quality of life by offering easy and relaxed access to kidney care. We believe skilled nursing facilities (SNFs) that adopt this model become more successful homes for their residents. Incorporating on-site dialysis helps forward-thinking nursing homes deliver a desirable quality of life.
