Introducing the Dialyze Direct Home Dialysis Program
“Can you do dialysis at home?” It’s a question many dialysis patients ask. Thanks to Dialyze Direct, the answer is now a resounding yes. With our Home Dialysis Program, patients can receive the care they need from the comfort of their own homes.
Preparing for Dialysis at Home
In response to the growing demand for in-home dialysis, Dialyze Direct is proud to launch the Home Dialysis Program—a supportive pathway for both dialysis patients and hospital case managers.
We understand the high volume of interest in home dialysis, and to meet this demand, we continue to expand our capacity. Our Home Dialysis Readiness Pathway is designed to keep patients engaged while they await full training or placement availability. This pathway provides access to periodic resources and updates, brief check-ins with Dialyze Direct staff to address questions and explore home dialysis options, and home-setting assessments for eligibility.
Once patients complete the readiness process and meet eligibility criteria, they will be added to the Readiness List and prioritized for the next available on-boarding slot.
Support for Case Managers
Our Pre-Enrollment Information Program works directly with hospital case managers to ensure they stay informed about program availability and have access to educational resources. This enables case managers to effectively advocate for home dialysis options for their patients. Case managers will also be enrolled in a priority update list and provided with materials and timely updates.
